I the undersigned allow and give permission for my child to participate and register in the Little Roos Soccer School Program run by Westgate Indoor Sports.
I understand that staff and coaches will at all times take full care and caution towards the children in their coaching techniques.
I am aware that this is a sports program and that injuries are common and may occur.
In any case, I will not hold coaches, staff and Westgate Indoor Sports responsible or liable should any form of injury or accident occur
Westgate Indoor Sports will take care but hold no responsibility for any loss or damage to personnel items before, during or after any of our coaching sessions.
Term fees are non-negotiable, non-refundable and cannot be paid weekly.
Term fees must be paid by no later than 3 weeks after first attendance, your child will not be allowed to attend the program, till done so.
If your child joins and there is less than 3 weeks left of the term then fees must be paid before the child's next attendance.
